As the legal guardian of the child or children participating in Flight Deck Gymnastics programs and activities, I acknowledge that serious injuries, including permanent paralysis or death, may occur in sports and activities involving height or motion, including but not limited to gymnastics, competitive gymnastics, tumbling, ninja, cheerleading, parties, and special events. Being fully aware of these dangers, I voluntarily consent to my child or children participating in any and all Flight Deck Gymnastics programs and activities and ACCEPT ALL RISKS associated with such participation.

In consideration of being permitted to use these facilities and participate in these activities, I, on behalf of myself, my child or children, and our respective heirs, administrators, executors, and successors, hereby COVENANT NOT TO SUE and FOREVER RELEASE Flight Deck Gymnastics and its coaches, instructors, teachers, employees, and representatives from all liability for any and all damages or injuries suffered by my child or children while under the instruction, supervision, or control of Flight Deck Gymnastics, including, without limitation, damages or injuries resulting from acts of negligence on the part of any coach, instructor, teacher, employee, or representative, to the extent permitted by applicable law.

In the event of an emergency, I authorize Flight Deck Gymnastics and its representatives to arrange for emergency medical care and transportation of my child or children to a hospital or other appropriate medical facility as deemed necessary. I agree to hold Flight Deck Gymnastics and its representatives harmless for arranging such emergency care in good faith. I further agree to be financially responsible for all medical expenses incurred by me or my child or children as a result of any injury sustained while participating in or attending Flight Deck Gymnastics programs or activities.

I acknowledge that I have read and understood this Assumption of Risk, Waiver of Liability, and Medical Authorization. I voluntarily agree to its terms.

I confirm that I have read and agree to the Flight Deck Gymnastics Assumption of Risk, Waiver of Liability, and Medical Authorization for all Children listed on this form.
